Workplace AI governance goes from rare to near-universal on paper, then gets routinely waived

A new survey finds 98 percent of large US companies now have a formal AI policy, but 47 percent admit to circumventing it for urgent deployments, and nearly two in five cannot say who is accountable for an AI agent once it is running.
